The membership-based model of Direct Primary Care offers unlimited provider visits, in-home options, 24/7 access to your medical team and longer appointment times.

Accessible pricing for diagnostic care

Vaccines, labs or radiology services can be billed through insurance or private pay. Because Elite Care patients are not required to carry health insurance, we offer our patients low cash prices for labs and radiology. We are able to do this by partnering with local businesses. Some FSA/HSA plans may also be used to pay for services. 

Monthly Membership Costs

Birth to 2nd Birthday

2 Years to 5th Birthday

Teachers, healthcare professionals and active military/1st responders receive 10% off all monthly membership fees!

Have multiple children?

Sibling Membership:

After your first child is enrolled, each additional sibling will receive a $150 membership discount from the price for their specific age group.

Example: A family with three kids over 5 years old would be $300/month total. $200/month for the first child, and $50/month for the other two children.

Office Only Option:

$125/Month per patient for all ages

Includes in-office and virtual/telehealth visits plus all membership benefits and services.

Note that house calls and sibling discounts are not included in this plan. House calls can always be added as a separate fee.

Non-member Costs

$150 per consultation: One-time Second Opinion Consultation (in-office or virtual)

$150 per visit: Non-Member Urgent Care Visit
